Are you living with pain? At Elite Physical Therapy & Sports Performance, we are dedicated to helping you identify the cause of your pain and creating a custom treatment program that will allow you to return to normal activities pain-free.

How you should prepare for your visit:

  • Arrive 15 minutes early for your first appointment.
  • Bring with you a copy of your insurance card and a photo ID.
  • Please call our office to verify your insurance benefits.
  • Make a list of questions that you have to discuss on your first visit.
  • List signs and symptoms that can be of importance to the therapist.
  • Have in writing key past medical history, previous treatments, current medications, and other important information.
  • Wear appropriate clothing – for lower body injuries, wear or bring a pair of shorts; for upper body injuries something like a tank top for access to the shoulders and neck.

Direct Access

Did You Know You Can See a Physical Therapist Without a Referral?

In the medical community, Direct Access is the ability to see a physical therapist when you want to without getting permission from your physician or medical practitioner.

Not everyone knows you can go directly to your physical therapist if you are dealing with pain or an injury. It doesn’t matter if you’re dealing with an old or new problem. In all 50 states, you can schedule a visit with one of our therapists to get help sooner than later.

However, in the state of Michigan there are some restrictions as insurance companies will require a referral from a physician to pay for physical therapy services. We request that you get a referral from your physician prior to starting physical therapy if you want to go through insurance.

Insurance Info

At Elite Physical Therapy & Sports Performance, there are a wide variety of different healthcare plans that we accept.  As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.

Do you have one of these insurance plans? We gladly accept the insurance plans listed below. If your plan is not listed, please call us at (231)421-5805 as we are always adding new insurance plans.

We have included most of the major plans with which we are in-network; however, this is not an all inclusive list. Please call the office to verify other insurance plans.

For those patients who do not have health insurance or those who have maxed out their benefits, we offer a private pay plan. Elite Physical Therapy & Sports Performance also offers a $75.00 per visit, self-pay fee. Please contact us for more information on our fees for service and payment details.

Do PT treatments hurt?

Physical therapy is not meant to hurt, but to relieve pain. However, as you restore your mobility, pain levels, at times can fluctuate. You might experience some temporary soreness from using muscles that are weakened or recovering.

This is a normal response to therapy treatment. Good communication with your therapist will help minimize any discomfort. We want you on your way to quick pain relief and back to normal activities.

What should I know about physical therapy?

Physical therapy is a medical, hands-on approach to relieving pain and restoring function. Physical therapists are medical experts in musculoskeletal and neurological conditions and work closely with physicians.

Our physical therapists evaluate your movement patterns, identify the cause of your pain, and work to eliminate it.

While every patient’s condition and pain is unique, our physical therapist will use a variety of gentle hands-on techniques, exercises, and equipment to relieve pain quickly and restore your function to optimum levels.
